If it is a new case, be sure that you have a Physician's Certificate of Medical Examination completed by a licensed Physician before your appointment. If you do not have a copy, please print the Certificate from the Forms tab drop-down menu. It must be less than 120 days old.
Wills, Statutory Durable Power of Attorney, Medical Power of Attorney, Supported Decision Making Agreement, HIPPA Release, Directive to Physician, Declaration of Guardian, etc.
Independent Administration, Dependent Administration, Muniment of Title, Affidavit of Heirship, Small Claims Estate, etc.
Be sure to tell the assistant the property address!
